Utility of 90Y-Ibritumomab Tiuxetan (Zevalin®) in the treatment of adult patients with non-Hodgkin lymphoma.

Con el objetivo de evaluar la utilidad del 90Y-Ibritumomab tiuxetan en el tratamiento de pacientes con Linfoma No Hodgkin (LNH), en términos de utilidad clínica y calidad de vida, comparado con el tratamiento convencional o la terapia con un anticuerpo monoclonal (AcMo), se realizó una revisión sistemática de la literatura disponible. Los autores concluyeron que la radioinmunoterapia con 90Y-Ibritumomab tiuxetan prolonga el tiempo a la recidiva y la supervivencia global de los pacientes con LNH indolente, aunque la información disponible en la actualidad es insuficiente para determinar la ratio riesgo/beneficio derivada de la inclusión de 90Y-Ibritumomab tiuxetan al estándar actual de tratamiento quimioterapia-rituximab.
Introducción
El 90Y-Ibritumomab tiuxetan es una de las últimas innovaciones terapéuticas incluidas en el tratamiento de los linfomas no Hodgkin (LNH) que expresen receptores CD20 en su membrana. Se trata de un anticuerpo monoclonal (AcMo) recombinante murino tipo IgG1 kappa específico para el antígeno CD20 de las células B marcado con Itrio-90 (emisor ß puro, alta energía 2,3 MeV, semivida de 2,67 días, penetración media 5,3 mm). Basa su efectividad terapéutica en el efecto citotóxico del AcMo al que se añade el efecto de la radiación emitida por el radioisótopo ligando. La unión específica del anticuerpo permite un tratamiento radioterápico localizado, minimizando el riesgo de efectos secundarios. La indicación terapéutica de este radiofármaco es el tratamiento de pacientes con LNH CD20+ que sean refractarias a otras líneas de tratamiento.

Objetivos
Evaluar la utilidad del 90Y-Ibritumomab tiuxetan en el tratamiento de pacientes con LNH, en términos de utilidad clínica (mejora del pronóstico, aumento del periodo libre de enfermedad o aumento en el tiempo de supervivencia) y calidad de vida, comparado con el tratamiento convencional o la terapia con AcMo sólo, a partir de la literatura científica disponible. Se persigue la identificación de ensayos clínicos en fase III.

Metodología
Se realizó una revisión sistemática de la literatura usando la red internacional de Agencias de Evaluación de Tecnologías (INAHTA), la Cochrane Library (2006, Issue 2), la National Guideline Clearinghouse, sitios WEB de agencias no incluidas en INAHTA y de instituciones internacionales y proveedores que elaboran informes para sus sistemas sanitarios, como Blue Cross and Blue Shield Association-Technology Evaluation Center, Institute for Clinical Evaluative Services (ICES), Medical Advisory Secretariat (MAS), National Institute of Clinical Excellence (NICE), páginas WEB de las Sociedades Científi cas de Hematología Medicina Nuclear y Oncológica nacionales e internacionales. Para identificar estudios primarios se procedió mediante búsquedas en las bases de datos referenciales MEDLINE (incluido PRE-MEDLINE mediante PubMed) y EMBASE (EVIDENCE BASED MEDICINE), así como en la base CENTRAL de la Cochrane Library, CINAHL, Clinicaltrials.gov, PsyInfo y LILACS, para localizar ensayos clínicos, adaptando en cada caso las estrategias de búsqueda a la base consultada. Se completó el proceso de identificación con una búsqueda manual en los documentos a texto completo seleccionados, de las listas de referencias bibliográficas, y las tablas de contenidos de las principales revistas relacionadas con el tema evaluado. Así mismo se analizó la bibliografía disponible aportada por la industria farmacéutica a fin de localizar todos los ensayos clínicos posibles.
La calidad de la evidencia científica se valoró mediante una escala específica en función del tipo de documento analizado (escalas del sistema de clasificación de la evidencia científica SIGN y por el CASPe.

Resultados
Se identificaron 15 ensayos clínicos fase III, de los cuales tan sólo 3 han finalizado. La búsqueda bibliográfica identificó 151 publicaciones potencialmente relevantes, que quedaron reducidas a 4 tras su cribado acorde a los criterios de inclusión y exclusión y la lectura a texto completo de las publicaciones seleccionadas. Se obtuvieron una revisión sistemática y tres artículos sobre los resultados de dos de los ensayos clínicos en fase III finalizados. Se consideran dos situaciones clínicas, la primera el uso de 90Y-Ibritumomab tiuxetan en LNH refractarios a tratamiento que fue la primera indicación terapéutica, y en segundo lugar el uso de 90Y-Ibritumomab tiuxetan en LNH como terapia de consolidación tras una primera línea de inducción.
En el primer caso, la tasa global de respuesta (TGR) para el grupo de pacientes tratados con 90Y-Ibritumomab tiuxetan es de 80% (68,1-87,7, IC 95%) y para el grupo control de 56% (43,4-67,4, IC 95%) con significación estadística de esta diferencia (p=0,002), La duración del tiempo la respuesta mostró diferencias porcentuales a favor del grupo 90Y-Ibritumomab tiuxetan, con significación estadística sólo a los 6 meses (p=0,046), La respuesta por subgrupos histológicos fue más marcada en los pacientes con linfoma folicular tanto en el TGR como en el tiempo de duración de la respuesta a favor del grupo 90Y-Ibritumomab tiuxetan. En los pacientes con LNH no folicular de bajo grado las diferencias no son tan evidentes.
En el segundo caso la supervivencia libre de progresión (SLP) en el grupo de tratamiento de consolidación con 90Y-Ibritumomab tiuxetan fue de 36,5 meses mientras que en el grupo control fue de 13,3 meses, con un riesgo relativo (RR) de 0,465 (p< 0,0001), Al considerar la tasa de respuesta se evidencia que la conversión de remisión parcial a remisión completa alcanza el 77% tras la terapia de consolidación frente al 17,5% en los pacientes del grupo control, La tasa final de RC/RCnc tras consolidación fue 87,4%, comparado con el 53,3% del brazo control. No se observan diferencias significativas en cuanto a la supervivencia global de ambos grupos.

Conclusiones
Existe un escaso número de ensayos clínicos fase III y publicaciones derivadas de la consecución de los mismos que evalúen la utilidad del 90Y-Ibritumomab tiuxetan como opción terapéutica en los pacientes LNH. La RIT con 90Y-Ibritumomab tiuxetan prolonga el tiempo a la recidiva y la supervivencia global de los pacientes con LNH indolente, aunque la información disponible en la actualidad es insuficiente para determinar la ratio riesgo/beneficio derivada de la inclusión de 90Y-Ibritumomab tiuxetan al estándar actual de tratamiento quimioterapia-rituximab. Sería recomendable la finalización de los ensayos que actualmente se llevan a cabo para confirmar los resultados preliminares obtenidos.
Introduction
90Y-Ibritumomab Tiuxetan is one of the latest therapeutic innovations in the treatment of non-Hodgkin lymphomas (NHLs) that express CD20 receptors in their membrane. It is a recombinant murine IgG1 kappa monoclonal antibody specifi c for the CD20 antigen of B cells, and marked with Yttrium-90 (a pure ß emitter with a high energy of 2.3 MeV, a half-life of 2.67 days and an average path length of 5.3 mm). Its therapeutic effectiveness is based on the cytotoxic effect of the monoclonal antibody to which is added the effect of radiation emitted by the bound radioisotope. The specific binding of the antibody allows a localised radiotherapeutic treatment, minimising the risk of secondary effects. The therapeutic indication of this radiopharmaceutical is the treatment of patients with CD20+ NHL that are refractory to other lines of treatment.

Objectives
To evaluate the utility of 90Y-Ibritumomab Tiuxetan in the treatment of patients with NHL, in terms of clinical use (improvement of the prognosis, increase of the period free of disease or increase in the survival time) and quality of life, compared with the conventional treatment or the therapy with MoAb only, using the scientific literature available. The identification of clinical trials in phase III is pursued.

Methodology
A systematic review of the literature and an economic evaluation were made using the International Network of Agencies for Health Technology Assessment (INAHTA), the Cochrane Library (2006, Issue 2), the National Guideline Clearinghouse, Web sites of agencies not included in the INAHTA and of international institutions and suppliers that produce reports for their healthcare systems, such as Blue Cross and Blue Shield Association’s -Technology Evaluation Centre, the Institute for Clinical Evaluative Services (ICES), the Medical Advisory Secretariat (MAS), the National Institute of Clinical Excellence (NICE) and the WEB pages of the national and international Scientific Societies of Hematology, Nuclear Medicine and Oncology.
Primary studies and clinical trials were identified by means of searches in the reference data bases MEDLINE (including PRE-MEDLINE by means of PubMed) and EMBASE (EVIDENCE BASED MEDICINE), as well as in the Cochrane Library CENTRAL database, CINAHL, Clinicaltrials.gov, PsycINFO and LILACS. In each case the search strategies were adapted to the consulted database. The process of identification was completed with a manual search in the full text of the selected documents, the lists of bibliographical references, and the tables of contents of the main journals related to the assessed topic. Also, the available bibliography contributed by the pharmaceutical industry was analysed in order to locate all the clinical trials possible.
The quality of scientific evidence was measured using a specific scale based on the type of document analysed (scales of the scientific evidence classification system of SIGN and by the CASPe).

Results
There were 15 clinical trials in phase III that were identified, of which only 3 have been completed. The bibliographical search identified 151 potentially relevant publications, which were reduced to 4 after their screening according to the inclusion, and exclusion, criteria and the reading of the full text of the selected publications. A systematic review and three articles were obtained on the results of two of the completed clinical trials in phase III. Two clinical situations were considered; the fi rst being the use of 90Y-Ibritumomab Tiuxetan in NHL refractory to the treatment that was the fi rst therapeutic indication, and the second being the use of 90Y-Ibritumomab Tiuxetan in NHL as a consolidation therapy after a fi rst line of induction therapy. In the fi rst situation, the overall response rate (ORR) for the group of patients treated with 90Y-Ibritumomab Tiuxetan was 80% (68.1-87.7, 95% CI) and 56% (43.4-67.4, 95% CI) for the control group with the difference giving a statistical significance of p=0.002. The duration of response showed percentage differences in favour of the 90Y-Ibritumomab Tiuxetan group, with statistical significance only to 6 months (p=0.046). The response by histological sub-groups was more marked in the patients with follicular lymphoma, as much in the ORR as in the duration of the response in favour of the 90Y-Ibritumomab Tiuxetan group. In the patients with low grade nonfollicular NHL the differences were not so evident. In the second situation, in the group with consolidation treatment with 90Y-Ibritumomab Tiuxetan the progression-free survival (PFS) was 36.5 months, whereas in the control group it was 13.3 months, with a relative risk (RR) of 0.465 (p< 0.0001). When considering the response rate, it was demonstrated that the conversion from partial remission to complete remission reaches 77% after the consolidation therapy as opposed to 17.5% in the control group. The final rate of complete response/unconfirmed complete response (CR/CRu) after consolidation was 87.4%, compared with 53.3% in the control group. Significant differences regarding the global survival of both groups were not observed.

Conclusions
There are few phase III clinical trials, and publications derived from them, which evaluate the utility of 90Y-Ibritumomab Tiuxetan as a therapeutic option in NHL patients. Radioimmunotherapy (RIT) with 90Y-Ibritumomab Tiuxetan prolongs the time to relapse and the global survival of patients with indolent NHL. However, the information available at the present time is insufficient to determine the risk/benefit ratio derived from the addition of 90Y-Ibritumomab Tiuxetan to the present standard chemotherapy-rituximab treatment. It would be advisable to wait for the trials currently underway to be completed to confirm the preliminary results obtained.
