Prenatal screening for aneuploidy using free fetal DNA in maternal blood. Systematic Review of the Literature.

Con el objetivo de valorar la eficacia, efectividad y seguridad de las técnicas de diagnóstico de aneuploidías usando ADN fetal libre en sangre materna, se realizó una revisión sistemática de la literatura. Los autores concluyeron que la secuenciación masiva en paralelo (MPSS) presenta validez diagnóstica como método de identificación y cuantificación de ADN fetal. El análisis de la asociación entre variables mostró una fuerte asociación negativa del peso materno con la fracción fetal y la fracción genómica del cromosoma 21 (valores z) y una fuerte asociación positiva entre fracción fetal y valores z. En el único estudio que evaluó el coste, se mostró una importante reducción de pérdidas fetales causadas por la instrumentalización de las técnicas invasivas y un posible gran ahorro económico en los embarazos de alto riesgo.
• Los métodos de diagnóstico de aneuploidías mediante ADN fetal libre en sangre materna, en embarazos de riesgo, podrían evitar la realización de pruebas invasivas como amniocentesis o biopsia de corion, disminuyendo las molestias y ansiedad que generan, así como posibles complicaciones en el parto causadas por la instrumentalización e incluso el riesgo de pérdida fetal.

• Los objetivos específicos de esta revisión se centraron en valorar la eficacia, efectividad y seguridad de las técnicas de diagnóstico de aneuploidías usando ADN fetal libre en sangre materna.

• Se recuperaron 3 documentos para su análisis, 2 estudios de pruebas diagnósticas y 1 estudio de casos y controles.

• Los estudios de pruebas diagnósticas verificaron la validez diagnóstica de la secuenciación masiva en paralelo (MPSS) como método de identificación y cuantificación de ADN fetal, con sensibilidad entre 79,1% y 100% y especificidad entre 97,9% y 99,7%. Estas pruebas mostraron mayor capacidad para descartar la enfermedad que para detectarla.

• Se obtuvieron valores similares al estimar la eficacia diagnóstica (curvas ROC de 1 y 0,98) de los dos protocolos de multiplicación empleados (2-plex y 8-plex). Sólo presentaron diferencias en los valores de z, significativamente mayores en el protocolo 2-plex (p< 0,001) y en el mayor porcentaje de cromosoma 21 identificado con el 8-plex (p< 0,001).

• Existió una fuerte asociación negativa entre peso materno y fracción fetal, así como entre peso materno y valores z, una fuerte asociación positiva entre valores z y fracción fetal en los casos (p< 0,001) y una pequeña asociación positiva entre edad gestacional y valores z. Con el resto de variables las asociaciones fueron insignificantes.

• No se encontró ningún estudio de evaluación económica pero uno de los trabajos incluidos calculó el coste del diagnóstico prenatal en embarazos de alto riesgo de aneuploidías, incluyendo la MPSS en el proceso antes de realizar anminocentesis o biopsia corial, mostrando una importante reducción de pérdidas fetales causadas por la instrumentalización de las técnicas invasivas y un posible gran ahorro económico.
• Prenatal screening for aneuploidy using free fetal DNA in maternal blood, in pregnancies at high risk, could avoid invasive diagnostic testing such as amniocentesis or chorionic villus sampling, reducing the discomfort and the anxiety they generate, and the possible birth complications caused by instrumentation and even the risk of fetal loss.

• The specific objectives of this review focused on evaluating the efficiency, effectiveness and safety of prenatal diagnosis for aneuploidy using free fetal DNA in maternal blood.

• 3 documents were retrieved for their analysis, 2 diagnostic tests studies and 1 case-control study.

• The diagnostic tests studies verified the validity of massively parallel shotgun sequencing like the identification and quantification DNA method, with sensitivity between 79,1% and 100% and specificity between 97,9% and 99,7%. These tests showed greater ability to rule out the infection than to detect it.

• Similar values were obtained estimating the diagnostic efficacy (ROC curves were 1,0 and 0,98) from the two sequencing protocols used (2-plex and 8-plex). They only presented differences in the z-score, significantly higher in 2-plex protocol (p< 0,001) and in the higher identification of percentage chromosome 21 in 8-plex sequencing (p< 0,001).

• There was a strong negative association between maternal weight and percentage fetal DNA and also between maternal weight and z-score values, an strong positive association between z-score and percentage fetal DNA only in cases and just a little positive association between gestational age and z-score values. The rest of the variables associations were insignificant.

• There was not economic evaluation study found but one of the selected studies calculated the prenatal screening costs in high risk pregnancies, including the MPSS in the process before the amniocentesis or the chorionic villus samplimg and showed an important reduction of fetal loss caused by the invasive techniques and a possible great economic saving
